Hours are Monday
– Friday 8:00 am
– 5:00 pmn n Job Descriptionn n Reporting directly to the Administrator and/or DON this detailed oriented and technically savvy individual delivers the best possible patient care by performing secretarial and other duties utilizing specific knowledge of medical terminology and home dialysis procedures.

Duties include compiling and recording and uploading medical charts, and preparing all necessary charts for home patients and skilled nursing facilities.n n Position Functionsn Assist the Administrator and DON with their day to day needs Function as Medical Record Coordinator Compiles and maintains medical records of dialysis patients according to company policies andnprocedures and in compliance with all applicable regulatory requirements.

Ensures patient confidentiality is maintained at all times.

Stores or disposes of obsolete files when appropriate according to the procedures defined by thenFreedom at Home Dialysis policy and in compliance with Federal, state and local regulations.

Prepares hemodialysis records for the following day’s activities ensuring they are stored/kept in ansecure location until needed per HIPAA requirements.

Charts, posts, and files appropriate information per defined procedures
– as needed Answer telephones and direct calls to appropriate staff quickly and courteously Greet visitors, ascertain purpose of visit and direct them to appropriate staff Operate office equipment such as voice mail messaging systems, and use word processing,nspreadsheet, and other software application to prepare reports, letters and case histories Able to assist in providing dialysis nurses their needs by pointing them to the right direction Receive and route messages and documents such as laboratory results to appropriate staff andnupload them in the EMR system as needed Compile reports and correspondences Transmit correspondence and medical records by mail, e-mail and fax Ensure that Dialysis Admit chart is ready for dialysis admission process Ensure that SNF Dialysis Binder is complete, updated and ready before scheduled SNF In-service Coordinate with HR that Field Clinicians have their badges and EMR Tablets ready prior to seeing thenpatient Perform various clerical and administrative functions, such as ordering and maintaining anninventory of supplies Maintains accurate records of hospitalization, patient travel, etc.

to facilitate coordination ofnpatient scheduling, ancillary testing, etc.

Assists in auditing records for ongoing compliance with medical records standards.

Assists with laboratory work process;n n> Prepares lab slips and tubes for routine and non-routine blood work;n n> Packs the box for shipment and then direct these to the appropriate laboratory as required.n n> Posts/enters the results from the labs into the electronic database (Devero EMR) as needed Other duties as assigned Qualificationsn n Education And Experiencen Previous Dialysis experience a Plus!

High School Diploma or equivalent Specialized training in medical record upkeep and HIPAA a plus Proficient in Microsoft Office, Word, and Excel 2+ years of experience in health care industry preferred Knowledge and experience of relevant software applications for communicating with fieldnstaff Proficient in spelling, punctuation, grammar and other English language skillsn n Soft Skill Requirementsn Excellent communication skills, both oral and written.

Ability to use discretion concerning sensitive information and adhere to HIPAA.

Ability to work on own initiative within clearly defined parameters.

Ability to carry out designated tasks including reporting outcome of actions taken.

Good organizational and time-management skills.
