Under the guidance of the Director of Clinical Services,the Clinical Social Worker/Program Therapist performs social services functions to include assisting patients in meeting their biopsychosocial needs while enabling them to achieve their optimal level of emotional health. The social worker/therapist is responsible for providing basic consultation, psycho-social assessment, individual, group, and family therapy, participation in treatment team planning, discharge planning/case management services, crisis intervention and liaison services with referral sources and other departments within the hospital as needed.The position is PRN/ flexible schedule positionKEY POSITION REQUIREMENTS* Formulate and conduct psychoeducational, therapeutic, and/or support groups as assigned* Communicate effectively with patients, families, health care team members, agencies, and community providers regarding patient status and progress* Complete Discharge Planning for patients* Assures that all components of Core Measures (HBIPS) related to HBIPS Tool are completed on the aftercare planIntervention* Understand and demonstrate the core elements of individual/couples/family counseling including theoretical framework, key concepts and techniques and skills* Accurately and thoroughly document interventions using goals developed in the master treatment planAssessment* Perform comprehensive psychosocial assessment on assigned patients within 72 hours of admission with consideration to age, mental, social and physical status and to include discharge-planning needs.* Inform supervisor of current activities on the unit and report unusual occurrences, incidents, or other problems promptly.* Assure that patient’s rights are upheld. Inform patient of grievance and appeal procedure and assists patient in processing concerns or issues.Treatment Planning* Actively participate in the development of the Master Treatment Plan by presenting pertinent information and conclusions identified through the psychosocial assessment within 72 hours of admission.QualificationsMINIMUM REQUIREMENTSEducation: Master’s Degree from an accredited college or university, in Social Work or a clinical related mental health field.Experience: Requires a minimum of one-three (1-3) years’ experience in a psychiatric health care facility, with direct experience in family and group therapies, crisis intervention, and treatment skills; must have strongclincal assessment skills. Combination of education, licensure and experience and/ or student internship hours may be considered.License: Must possess a state licensure: LMSW, LCSW, LPC, LPC-I, LCDC, or LMFTAdditional Requirements: BLS, CPI.
