1st Care Management is expanding and here’s an opportunity to become a part of the teamJoin our growing team of Nurse Practitioner’s, as we look to staff in new areas and other previously underserved regions.
Our NPs work on a contract (1099) and are PRN (as needed) based on their own schedules, and in the territories they are most comfortable with.
All the relevant training will be provided by our staff before you make your first visit.Buford/Sugar Hill/Cumming/Flowery Branch/GainsvilleAbout 1st Care Management:Our mission at 1st Care Management is to offer a new approach to patient care by eliminating the burden of office visits for patients unable to do so.
We promise care to our patients within the comfort of their own homes, on schedules that work best for them.By utilizing Transitional Care Management and home visiting practitioners, our collective goal is to decrease the patient’s risk of hospitalization.
We want to optimize their clinical outcomes through personalized and effective care.
Our Chronic Care Management program offers patients a support system to navigate through chronic conditions just like they would in a primary care setup, now from their own homes.
All of our services combined allow us to lessen our patient’s burden at a time when they need help the most.Job Responsibilities: Conduct in-home assessments and evaluations on patients.
This would include, but is not limited to:
•Past medical history
•Review of symptoms
•Checking vitals and conducting basic general physical examinations
•Medication review and reconciliation
•Depression screening
•Alcohol and drug abuse screening
•Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment
•Identify diagnoses to be used in care management and active medical management for purposes of treatment
•Recommend appropriate measures, including medication prescriptions and skilled needs for the patient
•Efficiently communicate findings in your assessment to the office/MD
•Educate members on topics such as disease process and prevention, medication, and compliance
•Comply with all HIPAA regulations and maintain security of protected health information (PHI)Required Qualifications:
•Current NP license, with no sanctions, restrictions, or provisions, in the state of Georgia
•At least 1 year of clinical experience
•Clinical background in adult, family or geriatric specialties
•Experience working with an Electronic health record (EHR)
•Computer literate
•Effective communication skills are critical
– ease of use with smartphone and email are required
•Reliable transportation
•Ability to work independently
•Willingness to spend at least 30 minutes to 1.5 hours with a patient in their homes, which may or may not be in the presence of pets, and/or family members that are tobacco users
•Able to navigate stairs and a variety of dwelling conditions and configurations
•Ability to sit, stand and kneel as needed to perform physical assessmentsPreferred Qualifications:
•Home care or home visiting experience
•Experience with EMR
