LVN Charge Nurse

This is a Full-time position in Tomball, TX posted August 29, 2021.

The LVN Charge Nurse is responsible for coordination and supervision of the assigned unit and ensures overall plan of care.

The Charge Nurse should follow the nursing care plan as established by the Unit Manager, but may make changes as care needs or physician orders change.

Job Responsibilities: * Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents in planning delivery of care.* Demonstrates knowledge, skills, and techniques necessary to care for residents with the following needs: therapeutic, neuro & dementia, dialysis, IV therapy, infectious disease, and wound care if applicable* Provides nursing care in accordance with Resident Care Policies and Procedures and ensures the safety and well-being of the residents is maintained.* Attends all mandatory in-services by employee’s anniversary date.* Maintains and updates Nursing license in complaince with Texas state regulations.* Demonstrates ability to adjust to changes in unit/shift assignments to meet resident and facility needs.* Makes daily rounds for all areas of nursing care under his/her supervision, visits each resident to evaluate physical and eomotional condition to receive comments relating to the resident’s needs and problems, and implements necessary nursing interventions.* Delegates responsibility to nursing personnel through work assignments, etc.

for direct nursing and non-nursing care of residents during the shift.* Demonstrates responsibility for administration of medications and treatments as prescribed by the attending physician and recording of such in the health record.* Ensures an adequate supply of prescribed medications are on hand and stored properly and all outdated or discontinued medications are disposed of according to facility policies.* Ensures that a thorough and comprehensive verbal and written report is given to the oncoming shift at the conclusion of duty* Ensures nursing units are clean and organized* Participates in resident care plan meeting as needed.* Ensures that ambulation occurs as an observed system* Turns, positions, and toilets residents according to established Care Plans* Ensures that a cleaning schedule is developed and implemented on each shift.* Conducts audits to assure the Pressure Sore Treatment protocol is being followed.* Manages resident meal times, including ensuring staff have completed lunch/breaks prior to resident meal times.* Observes and assesses total resident focusing on all body systems, sking integrity, weight, and hydration status and reports changes in resident condition promptly to the Supervisor and in writing on the 24 hour report sheet.* Is aware of, and adheres to, Resident’s Bill of Rights and Confidentiality of Resident Information.* Addresses family satisfaction issues immediately by reporting them to the Supervisor in a professional manner.* Demonstrates knowledge of and implements Universal Precautions.* Addresses residents in a respectful manner.* Additional duties as assigned by the Unit Manager, ADON, DON, or Executive Director.

Job Qualifications: * Currently licensed as a Licensed Vocational Nurse in the State of Texas.* Successfully completes facility-conducted orientation, mandatory training, and in-service programs.* Previous work experience in a long-term care setting is preferred.* Possesses excellent technical assessment and documentation skills, and leadership qualities.* Must be capable of performing the essential functions of the job, with or without reasonable accommodations.* Must be able to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required.
