H2 Health is looking for a Part time or PRN CFY or a CCC-Speech Language Pathologist for our H2 Health Pro Step located in Borger, TX. This position will consist of treating a pediatrics and adult caseload, ranging from articulation, dysphagia, dysarthria, autism, feeding, sensory issues, memory, cognitive impairments. We are looking for someone that can service about 10-12 hours per week.

Education: CCC-SLP Credentialed and/or Graduate of an accredited (SLP) Speech Language Pathologist program and currently licensed in the state of Texas.

Speech Language Pathologist – Job Description

Job Summary

Plans and administers prescribed physical therapy treatment program for patients to restore function, relieve pain, and prevent disability following illness, disease, or injury.

Patient Rights

  • Ensures that care is provided in privacy
  • Ensures protected health information is kept confidential
  • Reports complaints made by patients to supervisor
  • Reports allegations of patient abuse, neglect and/or misappropriation of patient property

Safety and Sanitation

  • Follows established safety and infection control policies and procedures
  • Observes safety needs of patients as indicated in care plan
  • Wears and/or uses safety equipment and supplies when indicated and properly trained to use
  • Demonstrates job-specific knowledge of fire and disaster preparedness during drills or actual situations

Staff Development

  • Attends and participates in scheduled in-service training, educational classes and meetings to maintain current credentials as applicable and as managed by regulatory agencies and company policies.

Speech Language Pathologist Responsibilities

Speech Language Pathologists completing their Clinical Fellowship (CF) require that all Speech Language Pathologist responsibilities are carried out under the supervision of a CF Supervisor

  • Reviews and evaluates physician’s referral and patient’s medical records to determine therapy treatment required.
  • Plans and prepares written plan of care on evaluation of available patient data.
  • Confers with physician and other health practitioners to obtain additional patient information, suggests revisions in treatment programs, and integrates physical therapy treatment with other aspects of patient care.
  • Records patient treatment, response, and progress.
  • Evaluates the patient and plans speech therapy services, as needed.
  • May instruct and supervise the clinical portion of training for students participating in speech therapist programs.
  • Diagnoses and evaluates speech and language competencies of individuals, including, but not limited to, assessment of speech, language, swallowing, cognitive and communication, and aural skills as related to educational, medical, social, and psychological factors.
  • Assesses the need for patients’ diet modifications and monitors changes on a regular basis, as appropriate.
  • Identifies patient conditions that may require intervention of a speech-language pathologist through screening processes.
  • Plans, directs and conducts rehabilitation plan of care to restore communication, cognition and swallowing abilities of individuals.

Supervisory/People Management Responsibilities

  • Provides input to the Clinical Director on performance appraisals for Rehabilitation Aids utilized during Speech Therapy
  • Supervises all clinical responsibilities delegated to Rehabilitation Aides participating in Speech Therapy, as appropriate per state practice act.

Administrative Responsibilities

  • Participates in department meetings and other meetings as assigned by Clinical Director.
  • Follows H2 Health Policies and Procedures
  • Maintains confidentiality of necessary information.
  • Performs any miscellaneous work assignments as may be required.
  • Assists the facility in assuring adherence to Federal and State regulations and certifications.

Regulatory and Certification Responsibilities

  • Assists the facility in assuring adherence to Federal and State regulations and certification
  • Actively participants in the regulatory or certification survey process and the correction of deficiencies

Education

  • Graduate of an accredited college or university with a master’s degree in speech language pathology. Must be qualified to practice in the state (licensed, registered or certified, as applicable) and in good standing with the Certificate of Clinical Competence (CCC) from the American Speech-Language-Hearing Association or completing clinical fellowship (CF) per ASHA guidelines.

Experience

  • Prior experience in field, preferred.

Physical Demands

  • Lifting 50 pounds maximum with frequent lifting and or carrying of objects weighing up to 25 pounds. Walking, standing, bending, stooping and sitting for prolonged periods. Full use of gross and fine motor skills of the upper and lower extremities. Possesses verbal and written communication skills. Ability to use senses in applicable tests and equipment use.

Travel

  • May provide services in a patient’s home or contextual environment including but not limited to a physician appointment, in-home intervention or home assessment, assist at other centers and/or attend company sponsored meeting.

Working Conditions

  • Inside with exposure to some disagreeable conditions and sometimes outside contracted events.
