1 | SUMMARY OF FUNCTIONSThe Pediatric Case Manager oversees and manages the clinical operations of the branch.

Is a liaison between the Agency, patient, and their families.

2 | MAJOR DUTIES AND RESPONSIBILITIESDevelops a plan of care that corresponds with the physicians orders through assessment of patient’s needs and consultation with other health team membersAssumes responsibility for continuity, quality, and safety of services delivered in compliance with State and federal regulationsAssists in proper maintenance of clinical and personnel records in compliance with local, state and federal lawsFollows all privacy policies and maintains the confidentiality of protected healthcare information (PHI)Provides supervision to all field clinical staffAssists to make sure branch establishes and meets quality of care goalsMonitors systems and identifies problem areas to Clinical Supervisor and Director of Nursing Operations both verbally and through written reportsAssists in investigating and reviewing initial employee and patient complaints as necessary and communicates to Clinical Supervisor and Administrator/Alt AdministratorMaintains professional knowledge and meets mandatory continuing education requirements of the agency/licensing board by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societiesPerforms other duties as required 3 | POSITION TYPE/EXPECTED HOURS OF WORKThis is a full-time position in office.

Days and hours of work are Monday through Friday, 8:00 a.M.

to 5 p.M.

This position regularly requires long hours and weekend work.

This position requires availability via telephone after regular business hours.

4 | TRAVELThis position requires working from office and visiting patient homes as needed for patient care oversight.

Overnight travel is not a regular requirement for this position, but may be needed occasionally.

5 | PREFERRED EDUCATION AND EXPERIENCEBachelor’s degree Nursing or related field.Pediatric Home Health ExperienceHome Care Administrator/Alternate Administrator Experience 6 | ADDITIONAL ELIGIBILITY REQUIREMENTSGraduate of an accredited school of registered nursing.

Diploma Program of Nursing, or Associate Degree of Nursing, or Baccalaureate Degree of Nursing or Baccalaureate Degree of Nursing, or higher-level degree of nursingCurrent Texas State Licensure as a Registered Nurse or Registered Nursing License from an approved Compact State with the Texas Board of Nursing (Will apply for Texas Nursing License within 30 days from date of hire) Communication, planning, time management, organization, and interpersonal skillsDemonstrated ability to supervise and direct professional, administrative, and direct care/clinical staffStrong team player with the ability to facilitate team building, develop effective partnerships and collaborate with a variety of stakeholders at all levelsAnalytical skillsMaintains high level of confidentialityProficient in Microsoft Office About Us: VIVA Pediatrics is a home health services company with a specialty in private duty nursing, therapy and skilled nursing visits for infants and children.
