Overview:Presbyterian Village North; Shift is from 7:00 AM 3:00 PMResponsibilities:ESSENTIAL DUTIES:Models a service and solutions oriented attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co-workersEnsure resident/patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazardsMaintain good attendance recordMakes beds, straightens rooms and keeps resident care areas in an organized and clean manner, ensuring the entire nursing floor is kept orderlyRespects resident’s rights by: (knocks on doors before entering room, addresses resident by name, allows resident to make own decisions when appropriate, promotes independence)Provides safe resident transfers involving bed, wheelchair and /or shower chair, uses the gait belt with all transfers, to protect resident from complications and injuriesTakes and records temperature, pulse, blood pressure and respirations as requiredAssists residents with simple range of motion or other exercises as directed by nurse or therapist and ambulates resident safely according to his/her endurance levelsApplies medical or adaptive devices as ordered by the appropriate supervising nurseMakes rounds to ensure resident’s safety, comfort, medical attention every hour or as required by the residentCollects specimens as directedCompletes weights for each resident as requiredSets up meal tray and assists with feeding as necessary.

Assesses for choking and responds appropriatelyEnsures residents and equipment are clean after meals, including hands, and removes clothing protectorsAccurately documents care given to residents including the documentation of reportable resident problems (i.E.

ADL sheets, B& B sheets, meal percentages, I& O, s/s of skin breakdowns, etc.)Reports changes in resident condition to the supervising nurse and /or Director of Nursing immediately (i.E.

changes in skin conditions, urinary changes, etc.) Reports problems with completion of assignmentsHandles confidential information appropriatelyDemonstrates flexibility with changing work demandsReports to charge nurse: equipment and supply needs, repair/maintenance needs, safety hazards and unusual occurrences or observationsAttends and participates in appropriate in-service and staff meetings to improve knowledge and skills and to maintain competency and certificationWears prescribed uniforms, follows prescribed dress code, wears name tag appropriately and maintains a professional image to the public and residentsAssists the nursing department and other departments when required in other duties and tasks when askedProvides appropriate interventions for resident behavior per care plan, utilizes distraction and validation techniques as appropriate and encourages resident independenceAssists residents to and from activities of choice and encourages the resident to participate in activities of choiceAssists activity director as needed with activities and if necessary leads activityOther duties as assignedQualifications:MINIMUM QUALIFICATIONS:Education: High school graduate or GED PreferredPossession of a valid Texas Certified Nurse Assistant certificationAbility to read, write and effectively communicate in EnglishSix months experience in providing personal care in a long-term care facility preferredKnowledge/ability to learn computer skillsAbility to work independentlyAbility to adjust to changing conditions, frequent demands and stressful situationsAbility to acquire new knowledge or perform additional or more difficult tasksAbility to maintain a positive, stable disposition in performing, working in association with staff and residentsAbility to maintain a high level of performance in an environment of frequent demands and changesAbility to work effectively as part of a healthcare team, to share information freely, to use and seek supervision and consultation verbally and in writing, and to be able to follow both verbal and written instructionAbility to perform Essential Duties as outlined belowGenuine caring for and interest in the care of mature adults, handicapped people and hospice PatientsAbility to perform work tasks within the Physical Demand Requirements as outlined belowAbility to comply with the Resident Bill of Rights (LTC) and Employee ResponsibilitiesAbility to comply with organizational and departmental safety policies and procedures
