SummaryResponsible for the delivery of Whole Health (WH) coaching & skill development of the Medical Center’s WH coach program, including provision of care through coaching/promotion/education regarding WH coaching to Veterans & staff.

Supporting WH coaches/Veterans in navigating the WH journey, through the Pathway, wellbeing & WH clinical care elements of the WH system, assisting in the collection of WH qualitative data from Veterans.One position located in Corpus Christi TX & one in Laredo TX.

Learn more about this agency ResponsibilitiesDuties include, but are not limited to:Health Coach:Contacting eligible participants for enrollment and explain the health coaching processProviding health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy onesConducting interventions for Veterans using one-on-one, couple, or group modalities in outpatient and inpatient settingsConducting Whole Health Facilitated GroupsParticipating in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health, and other inpatient and outpatient groups as appropriateProviding follow-up care through telephone clinics or secure messaging between face-to-face appointmentsActively guiding Veterans in the exploration of core elements of personal health planning through individual and group based programsEncouraging and assisting Veterans with the adoption of habits that are conducive to a higher quality of life and in alignment with the Veteran’s goalsIdentifying habits and behaviors that are harmful to quality of life and related to chronic illnessDeveloping treatment plans for Veterans focused on behavioral interventions based on current scientific knowledge and research that improve quality of life and reduce risk for chronic disease, or that reverse or slow progression of chronic diseaseUtilizing motivational interviewing and health coaching techniques and processes, learned through prerequisite health coach trainingFostering an environment of individual responsibility to maximize each Veteran’s ability to adhere to a lifestyle management planWorking collaboratively within a team setting to provide positive outcomes for participants, peers and overall operations.Coordinating with others on the Veteran’s health care team to ensure optimal patient careProgram DevelopmentDeveloping and distributing locally approved health education materials to identified enrolled Veterans and family members.Serving on Healthcare System, VISN, and/or National VA committees or task forces specifically relating to Whole Health or Health CoachingRepresenting Primary Care or Patient Centered Care/Whole Health on decision-making committees within the Healthcare SystemAssisting with data management analyses, special studies, and reports for use by management for budget preparation, workload projections, and quality assurance and utilization reviewsParticipating in the establishment of administrative standards locally; the objective of which is intended to improve programs, facilities and operationsMaintaining a close working relationship with various program officialsPerforming other related duties as assignedPromotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met.

The Individual Occupational Requirement for the Social Science Program Coordinator position is:Degree: successfully completion of a full 4-year course of study in an accredited college or university leading to a bachelor’s or higher degree that included a major field of study or specific course requirements in behavioral or social science; or related disciplines appropriate to the position.

OR Four years of appropriate experience that demonstrates you have acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field.

This experience must must include one year of specialized experience.

OR Combination of education and experience that provided the knowledge of one or more of the behavioral or social sciences equivalent to a major in the field.

Twenty four semester hours will be considered as equivalent to a major field of study from an accredited college or university.

Creditable experience should demonstrate appropriate skills or abilities needed to perform the work of the Social Science Program Coordinator position.Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement.

For a GS-07 position you must have served 52 weeks at the GS-05 and for a GS-09 position you must have served 52 weeks at the GS-07.

The grade may have been in any occupation, but must have been held in the Federal service.

An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials.

If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement.

In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade.Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment.You may qualify based on your experience and/or education as described below:GS-07Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization.

Examples of specialized experience would typically include, but are not limited to: determining ways to improve customer satisfaction by utilizing knowledge of the issues, program goals and objectives, work processes, and administrative operations of the organization; utilizing interpersonal skills to communicate and present recommendations that improve patient services and providing information and advocating for an individual’s personal health goals and plan, and sharing goals with clinical care team by partnering with individual in completing a Personal Health Inventory and exploring aspects of a Personal Health Plan in group and individual settings.

To qualify based on education for this grade level you must have one year of graduate-level education or superior academic achievement.
