Work Hours/Shift:


You Will Be Responsible For:

  • Responsible for operating all aspects of the facility vehicle safely.
  • Schedule service as maintenance dictates.
  • Maintain all documentation related to the facility vehicle to be sure that information is valid and current.
  • Inspect and certify the hydraulic life annually.
  • Complete the transportation log after each transport.
  • Wash and clean the interior as needed.
  • Safely unload, load, and secure residents for transport.
  • Appropriately accesses and safeguards residents’ medical records for the purpose of scheduling medical appointments.   
  • Schedules resident’s medical appointments outside the facility.  Maintains and updates the appointment book.
  • Arranges appointment transportation.
  • Applies for public transportation assistance, if applicable
  • Documents transportation arrangements in the appointment book.
  • Assists in the preparation of laboratory slips. Check refrigerators for lab specimens and provide couriers with specimens on pick up.
  • Accepts on-call status and is available to respond to the facility’s transportation needs timely.
  • Gives general care such as bathing, dressing, shampooing of hair, nail trimming, cleaning of teeth, hair care, and shaving of residents or assists residents to do their own care within limitation. 
  • Responsible for making and changing beds.
  • Attends to resident’s personal hygiene needs as required.  May assist residents to and from the bathroom and give bed pan.
  • Serves meals to residents and assists those who cannot feed themselves or need assistance with their meals.  Records food and fluid intake per policy and procedures.
  • Performs personal care and services to meet individual resident needs and preferences. 
  • Communicates to enhance dignity and respect. 
  • Answers call lights as required, keeping in mind that employees must respond to all calls regardless of who has immediate responsibility for the resident.
  • Reports any change of condition or unusual findings to the unit nurse per policy and procedures.
  • Applies aseptic technique in all resident care.  Follows Universal Precautions.
  • Assists in transfer maneuvers, using proper body mechanics and gait belts per policy.
  • Documents required residents’ information as outlined by the facility policy and procedures.
  • Checks and records vitals and weight on a regular basis following established policy or at the request of the unit nurse or at own discretion if the need exists.
  • Carries out restorative nursing measures.
  • Attends staff education, training and other required meetings.
  • Follows the Nurse Aide Standards of Practice.
  • Follows the residents’ care plan and communicates with the nurse about changes.
  • May come in contact with blood and other infectious body fluids.
  • Attends staff education, training, and other meetings
  • Maintains professional conduct while interacting with residents
  • Follows facility policies and procedures including but not limited to resident rights, infection control, dignity, etc.

What You Will Need:

  • High School Diploma or Equivalent
  • Must be 18 years of age or older
  • Valid state driver’s license in good standing
  • Current Basic Life Support (CPR) certification from a certified instructor requiring live return demonstration, such as the American Heart Association is preferred, and required to be obtained within the first 30 days after employment
    • Current Certified Nursing Assistant License in the state hired; or meets the state-specific regulations pertaining to reciprocal or post-exam licensure; and obtains state license within first 120 days of employment. 
  • Proficient in the use of a compute

Job Summary:

The Transportation Supervisor is responsible for all aspects of operating the facility vehicle safely.  They are also responsible for assuring that it is maintained and clean. The Transportation Supervisor schedules of residents’ outside medical appointments and communication of appointments to the resident and appropriate employees. Additionally, The Transportation Supervisor assists professional nursing personnel in providing nursing care and simple technical nursing services under the direction and supervision of a registered nurse or licensed vocational nurse
